Information to Start the Process
There are four main steps:
1) Ensure that you have a qualifying condition AND have medical documentation that states or indicates that diagnosis (here's the list of qualified conditions) --it is required that you have documentation of that history.
2) Schedule your appointment. The visit is 1 hour for an individual, 2 hours if you have a caregiver. That being the case, evaluations are by appointment only.
3) Complete your short Department of Health online application.
4) Come in for your appointment.

The actual Medical Marijuana Registry Program application is completed through the Department of Health's (DOH) portal. Here is the link to start that process http://health.hawaii.gov/medicalcannabisregistry/application-information/

There are two parts, the patient registration, and then the provider certification section. Once you are done with your portion of the application, our certifying provider is able to review the application.
It is important to note that you MUST be at least 18 to be evaluated at our clinic.
Once you complete the online application, it will ask for $38.50. This fee goes straight to DOH, it does not come to us and is not refundable from the State. The online portion should only take about 10 minutes to complete, and must be done prior to coming in for your appointment.
Our certifying fee is $195 (that payment is collected at the time of your appointment after your evaluation --we accept cash, credit card, PayPal, debit with the MC or Visa logo, no checks please). If your condition is not qualified, then we will not charge you for the visit. If you come back to our clinic the following year the price is reduced to $165 --we love being able to build a relationship with our clients to better address their health and wellness goals.
Please make sure to bring your state ID. Out of state ID is fine, but you must have a local physical address in order to submit your application. Also, if you are taking any prescriptions please bring the complete list and doses with you when you have an appointment.
